It depends on the book to an extent. I wouldn't have had the design skills to create mine, nor the budget to take the risk on funding the production and print run. It would have been a very different project doing it alone. Also, relevant to your point on publishers become more selective, there is an article in this week's Economist about that, due to rising paper and energy costs.
